Reach Community Outreach Project
James Bay New Horizons and Silver Threads Service are working in partnership to reduce isolation of seniors. The concept is simple – have Outreach Workers meet with seniors where they live, to make connections, listen to concerns and inform seniors of available services to help them remain independent. Our staff provide on site programs as well as one on one support, where you live.
We are pleased to have received funding from the Federal Government New Horizons Grant to provide Outreach Services to the senior community in partnership with the District of Saanich and community partners.
Project Goals
- To connect seniors to services and resources to improve the quality of life
- To connect seniors to their neighbours
We do this by holding Information/Coffee Parties on site to meet the residents and discuss their needs. Follow up support provided includes one on one support and as well as potential regular on-site health, wellness and social programs.
An information session and social is typically held in the common room or lounge of a residential building. The session lasts between 1 hour to 1.5 hours. We begin with a brief presentation of resources available for seniors, and participants are provided with a resource package containing this information and much more to take home. There is also time for discussion and questions, and the outreach worker offer a follow-up call to have individual conversation with participants should they wish to discuss anything in further detail, with the goal of connecting seniors to the services and programs that will suit them, as well as simply offering a listening ear.
